问题到底在哪里？

今天到学校办退学手续，下午收到老师的学习报告。让我很吃惊的是为什么短短半年老师的评价改变这么大，究竟是哪里除了问题：
11/30/2010 Angela is such a sweet girl! In the beginning of the school year I used to have to remind her about sitting nicely and keeping both feet on the ground instead of one the chair but she is doing very well now. Sometimes she likes to talk and play with the kids next to her so pls remind her of the rules at home. Please remind her that she needs to listen and look at the teacher when she is in class and not play with her clothes or other people in class. Angela only knows a few of the words, please continue to practice with her at home. When I ask her to identify a word, she usually waits for me to tell her the answer or for another student to say  it for her.
06/25/2010 Angela has done a wonderful job this year in English class. She has consistently been one of the top students in my class this year. What a greet way to start her educational life. I hope she continues to do well in english and other subjects she will learn in school. I have really enjoyed being Angela's teacher this year.
12/31/2009 Angela is a super student. She is very quick to pick up new words and very quick to understand instructions. Angela has done very well with the supertots material. Angela is very confident. She is always willing to try anything. Pls encourage Angela to keep up the good work.
这个学期一直反复生病，每天早上都是在吵闹中结束，舞蹈课也不想去。问题到底在哪里？
我很焦虑也很着急。
